Abstract

The number of outpatient follow-up visits, participation in a supervised exercise regimen, and the frequency of participating in a supervised HEP during the first 15 days (as a group) significantly predict readmission that may occur during the 16 to 30 days following hospital discharge. The more frequently patients perform their HEP, the less likely they are to be readmitted during the 16 to 30 days following hospital discharge. Therefore, it may be recommended that individuals with acute decompensated heart failure (ADHF) make appointments within the first 15th days of returning home before being discharged from the hospital to prevent interruption of care coordination.
